Healthy Perks:
CaliforniaChoice – offers three discount programs in conjunction with your Medical Plan included at no cost to the employee per month from payroll. These are discount programs and not associated with your Medical insurance. We offer a discount hearing program, discount vision program and discount dental program.
These discount programs give you dollars off the normal services by showing your card – ie $5.00 eye exam at the list of providers for that plan. Here are the summaries of these discount programs:
When the member enrolls, Cal Choice sends out two ID cards. The Medical ID and the Value Plus Enrollment Card.
Value Plus Enrollment Card:
To access these benefits, you will need to show your Value Plus Enrollment Card which was sent with your Cal Choice ID Card. This is a separate benefit from your Medical insurance. (If you can not locate your card, please contact Cal Choice and request a duplicate).
