For existing employees, enrollment for the Voluntary Life/AD&D is available anytime during the year for yourself and your eligible dependents but Evidence of Insurability (EOI) is required for any amounts over Guarantee Issue or if enrollment is requested in any amount outside of an employee’s initial eligibility period. New hires and their dependents may elect up to the Guarantee Issue amount without EOI. See policy summary for full details.
