Leave of Absence

In general, employees file a leave for absences greater than 5 business days due to:
• Employee’s own medical condition, including Pregnancy (continuous or intermittent)
• Care for a newborn, newly adopted or placed foster child
• Care for a seriously ill family member (continuous or intermittent)
• Extended Leave without Pay – Personal Leave
• Military Leave

To apply for a leave of absence, employees must:
• Notify your supervisor, providing expected start date of leave and anticipated return date as soon as possible.
• Contact Human Resources email: Listleaveadmin@ccah-alliance.org with as much notice as possible.
Notification of at least 30 days in advance is recommended when foreseeable. If not, as soon as possible thereafter.
