Specialty Care Management (SCM) Overview
What is Specialty Care Management (SCM)?
A bilingual care program designed to educate – anyone who is enrolled in Amy’s medical plan – on how to prevent and manage chronic health diseases and, if qualified, support with care.
Awareness Program: SCM provides various educational materials about weight management, healthy blood pressure, diabetes, and cancer prevention/early detection.
- SCM Registered Nurse outreach: Qualified members will receive a nurse outreach by phone in phases for help with Chronic Kidney Disease (CKD) prevention and management and Active Cancer Management.

- Chronic diseases are common, and along with other Amy’s benefit programs, SCM is now here to support, prevent and manage these conditions with you for the best outcome.
Who is eligible for this benefit?
- You and eligible dependents who are currently enrolled in our medical Plan. This benefit is 100% confidential, and there is no cost to you.
